Telling a patient and family that everything went well can turn fear into relief in seconds. It is one of the most meaningful moments of my work. Then doctors started electroconvulsive therapy (ECT). After three daily sessions, she recognized her family again. Days later, her hallucinations and severe agitation had resolved. Across more than 75,000 patients, women were less likely to receive opioid pain medication in the ER and during transport, less likely to be sent home with pain medication, and waited longer when treatment was given. Even when women and men reported the same 9/10 pain in the exact same medical scenario, doctors and nurses rated the woman’s pain lower. Women were more likely to be viewed as exaggerating or steered toward psychological treatment, while men were more likely to receive pain medication. And this wasn’t simply male doctors treating women differently. The same pattern appeared with female clinicians. I trained with a great surgeon who would leave long operations for 15-minute cigarette breaks while the entire surgery waited for him to return. Decades ago, some surgeons simply smoked right in the OR, avoiding the downtime but obviously creating other problems. I’d want my surgeon completely focused on my operation, not thinking about their next cigarette. But is a smoking break really different from a surgeon taking a snack or bathroom break?
