Early in my training I faced a situation in the operating room that created a real ethical dilemma. The surgery itself was going perfectly. The surgeon was highly skilled. But something about the situation kept bothering me.

When you’re standing inches from someone in an OR, you notice things. And sometimes those moments force you to ask difficult questions about responsibility, judgment, and what the right thing to do really is.

This happened over 20 years ago, but scenarios like this are still discussed in medicine today because there isn’t always an obvious answer.

I’m curious how people think about situations like this.

A patient came in with severe abdominal distention and hadn’t been able to go to the bathroom for weeks despite every treatment.
When we positioned him face-down for surgery, the pressure released instantly… and the entire operating room froze.

It was one of the most shocking moments I’ve ever witnessed in an OR.

I was relieved for the patient… but I felt terrible for the medical student standing at the foot of the table.

A patient needs brain tumor surgery. But planning A patient needs brain tumor surgery. But planning the operation requires an MRI so doctors can see the tumor’s details.

There’s one major problem.

The patient refuses to remove the metal body art covering his face. And these metal implants make MRI extremely dangerous.

Now the surgical team faces a difficult decision. Delay surgery, move forward without the MRI, or try to persuade the patient to remove it.

Cases like this raise a real question about medicine, risk, and patient choice.

But your instinct says something is very wrong.

If that cut is a mistake, the consequences could be catastrophic immediately. 

Moments like this are some of the hardest decisions in medicine.
Do you follow the order… or stop and challenge it?

“Oh crap.”

But the real question is bigger than the shock.

How does something like this even happen?
Was it the implant? The bone? Something that changed after surgery?

You don’t need medical training for this one. Just look closely.

Something almost impossible can happen during life Something almost impossible can happen during lifesaving efforts.

A person who has lost normal heart function suddenly wakes up. They can talk, move, even try to push the rescuers away because the compressions hurt. But the heart still is not working on its own, and the only thing keeping blood moving to the brain is the treatment being performed.

This rare phenomenon is called CPR-induced consciousness.

It creates one of the most difficult ethical moments in emergency medicine. If someone appears awake but their heart still is not functioning, should the team stop because the person is asking them to… or continue because it may be the only way to keep them alive?
